I, {name} entirely upon my own initiative and risk and responsibility am about to participate in training of Brazilian Jiu-Jitsu, Muay Thai, MMA training or combination of the before mentioned, at Unity Jiu Jitsu Grand Rapids, with all its appointed coaches. A parent or legal guardian must sign on behalf on anyone under 18 years old.

In consideration of being allowed to participate in any way in the martial arts training, sports programs, and other events and activities conducted by Unity Jiu Jitsu Grand Rapids the undersigned participant:

Certifies that the participant is in good physical condition and has no disease or injury that would prevent or impair participation in this activity, and that no instructor, coach, manager, doctor, nurse, medical professional, athlete, trainer, or other person has advised the participant not to participate in a contact sport/activity for any reason.

Acknowledges and fully understands that each participant will be engaging in activities requiring physical contact, that by their very nature involve risk of serious injury, including permanent disability, paralysis, and death. Mishaps such as falls, equipment failure, other participants or instructors or guests, contact with various hard and soft floor or ground surfaces, other people, and/or over exertion could result in injuries including sprains, concussion, broken bones, torn muscles/tendons, dislocations, puncture wounds, psychological trauma, hospitalization, blindness, and death. Knowing all of the above, I freely choose to participate in the above refereed activities and acknowledge my risk.

In case of emergency, I give my permission to receive any first aid, transportation or medical attention that may be required. 

Therefore, I do hereby for myself, my heirs, executors, and administrators release and hold harmless waive and release Unity Jiu Jitsu Grand Rapids, as well all its appointed coaches, CFS Fusion LLC and its board members, and any guest instructors from any and all responsibility, liability, negligence, claims, demands, actions or causes of actions, or caused of actions resulting from any participation in this sport/activity/class and associated activity.

By my signature, I indicate that I have read and understand this Waiver of Liability. I am aware that this is a waiver and a release of liability and I voluntarily agree to its terms.

I, hereby grant permission to Unity Jiu Jitsu and Unity Jiu Jitsu - Grand Rapids to reproduce any portion of the photo/video images that have been taken of me for the purpose of USE and or PROMOTION publications which can include but is not limited to, books, cards, calendars, invitations and websites without any compensation or recognition given to me. Furthermore, I grant creative permission to alter the photograph(s). I do not grant permission to resale or use the photographs in a manner that would exploit or cause malicious representation toward me or my company and associates.
